What is an Access Code?

The key to viewing the Care Plan prescribed to you by your provider relies on two pieces of information:

• Access Code
• Patient's Date of Birth

While you can create a MYSelectPT Portal account without this information, you will not be able to view your Care Plan until these have been entered into your account.

There are three ways in which you can receive your one-time use Access Code:

• In a Welcome Email                                                     
  
• On a Care Plan Handout                             
  
• Directly from your therapist or the clinic
